Insurance Accepted

We have provided this list as a guide, although please be aware that your insurance is a contract between you (or your employer) and your insurance company. It is the patient’s responsibility to verify that our providers are in network with your insurance company. Please contact your insurance company if you are unsure or have questions about your deductible or co-insurance. We are available to care for your dermatology needs, and will file your consultation and any accompanying procedures with your insurance company; you are responsible for the costs related to your healthcare.

Office Guidelines

New Patients
New patients can access new patient packet here and notice of privacy practices here, and bring them completed when they arrive for their appointment.

It is recommended that new patients arrive at least 15 minutes early for their appointment in order to complete patient information forms.

Late Policy
In the case of a late arrival, the patient may not be seen by the physician or nurse. It may be necessary to re-schedule the appointment for a later date.

We kindly request that you give us 24 hours notice if you are unable to keep your appointment. Failure to give 24 hours notice could result in a $35.00 missed appointment fee. This fee is not covered by your insurance plan.

Insurance and Payment Information
We are Medicare participating providers, therefore we will bill Medicare directly. You will be responsible at the time of service for payment of:

  • The annual deductibles
  • Copayments
  • Charges for non-covered or cosmetic services

You will be asked to sign a Waiver of Liability Form in the event that a service is provided, which we know is not
covered by Medicare.
If you have no health insurance, payment is expected in full at the time of service. Patients are responsible for payment of services and the practice is considered an Out-of-Network provider for PPO insurance policies. Therefore, patients with PPO insurance can file for procedures which may be covered under their policy. For your convenience we accept cash, check, Visa, Mastercard, Discover, and American Express.

Cell Phones
Please respect other patients and kindly turn off cell phones when you are in the BV Dermatology office.

Prescription Refills
Contact your pharmacist or contact our office for assistance with refills.
